PROPOSAL OBJECTIVES
MFH requires up to three (3) health-related,
measurable and time-specific objectives that relate directly
to the requested funding. Proposal objectives are a critical
component of successful projects and therefore are carefully
analyzed. These objectives will be used to evaluate (1) progress
during the project and (2) overall achievements at the end
of the project. Objectives not meeting MFH's requirements
will result in a declination of a proposal.
Objectives should be realistic in terms of time,
resources applied and other factors that influence their achievement.
Corresponding activities, measures and outcomes of the project
are explained in the proposal narrative.
Applicant objectives must answer the following:
-What will happen
-For whom (target population)
-How many (a statement of percentages or numbers
that either increase or decrease from a baseline as a result
of the project)
-When (time-specific achievements expected during
the project)
EXAMPLES OF OBJECTIVES
1. Within 6 months of grant award (when), as
a result of establishment of a shared management arrangement
between two primary care providers, costs for administrative
services (what) will be reduced by 15% from baseline costs
of $225,000. (how much) at Family Care, Inc. (who).
2. Within 9 months of grant award (when), shared after-hours
on call medical coverage (what) will have increased the volume
of low-income patients (who) seen by the participating clinics
by 25%, from 200 annually to 250 annually (how much).
3. Within 12 months of grant award (when), a shared billing
system implementation (what) will reduce billing costs by
14% from baseline, from $75,000 annually to $64,500 (how much)
at Family Care, Inc. and Medical Care Plus Health Centers
(who).
