and health care system strategies to address a variety of

Information Available: A systematic review of the effectiveness
of selected population-based interventions addressing diabetes.
The chosen programs fit within two strategic areas: health-care
system level interventions and/or diabetes self-management.

Description: The Diabetes Control Program,
a public health program in the California Department of Health
Services, works to prevent and detect diabetes, improve quality
of care, develop and sustain public health policy, reduce
health disparities and raise diabetes awareness.

facts and figures, publications-including a 2002 Basic Guidelines
for Diabetes Care Packet (includes essential components of
diabetes care, offers evidence-based recommendations), statistics
and links to other diabetes resources.

National Governor's Association (NGA) Center for Best Practices

Description: The NGA Center for Best
Practices tracks state efforts to prevent and manage diabetes.
The Center provides research and analysis for states on emerging
issues and state best practices.

site offers authoritative, up to date and appropriate health
information for both health professionals and consumers.

publications from the National Institutes of Health, general
information, alternative therapies, diabetes journals and
newsletters, clinical trials, symptoms, prevention programs,
research, statistics, law and policy, specific populations
and reports on the management of diabetes.

Description: The National Institute of
Diabetes and Digestive and Kidney Diseases conducts and supports
research on many of the most serious diseases affecting public
health. The Institute's Division of Intramural Research encompasses
the broad spectrum of metabolic diseases such as diabetes,
endocrine disorders, digestive diseases, nutrition, renal
disease and hematology.

of Diabetes Educators is the leading authority in Diabetes
Self-Management Training (DSMT) and in lifestyle management
for the prevention of diabetes. The organization focuses on
the goals of diabetes recognition, education, advocacy and
research.

statistics, a legislative advocacy center and information
about conferences and public educational campaigns.

Missouri Organizations:
Kansas City - Chronic Disease Coalition (KC-CDC)

Description: The KC-CDC is a community-based
network that examines ways to promote health within the neigborhoods,
churches and the community of Kansas City, Missouri. The KC-CDC
works to address the high prevalence of diabetes and cardiovascular
disease among African Americans and Hispanics. The organization
hopes to create environmental conditions that promote widespread
behavioral changes and improvement in population-level outcomes
associated with the high prevalence of these diseases.

St. Louis Diabetes Coalition (SLDC)

Description: The SLDC was created to
address the enormous challenge of diabetes awareness and care
through an integration of efforts and resources. Representatives
of SLDC include: managed care organizations, physician groups,
state and community health programs, the American Diabetes
Association, academia, business and professional organizations,
hospital groups and pharmaceutical industry leaders.
