The Community Guide

Description: The Community Guide, developed by a non-Federal Task Force on Community Preventive Services, offers leadership in the evaluation of community, population and health care system strategies to address a variety of public health and health promotion topics.

Information Available: A systematic review of the effectiveness of selected population-based interventions addressing diabetes. The chosen programs fit within two strategic areas: health-care system level interventions and/or diabetes self-management.


Diabetes Control Program

Description: The Diabetes Control Program, a public health program in the California Department of Health Services, works to prevent and detect diabetes, improve quality of care, develop and sustain public health policy, reduce health disparities and raise diabetes awareness.

Information Available: Successful diabetes projects, facts and figures, publications-including a 2002 Basic Guidelines for Diabetes Care Packet (includes essential components of diabetes care, offers evidence-based recommendations), statistics and links to other diabetes resources.

Description: The NGA Center for Best Practices tracks state efforts to prevent and manage diabetes. The Center provides research and analysis for states on emerging issues and state best practices.

Description: The goal of the Division of Diabetes Translation is to reduce the burden of diabetes in the United States. The division combines support for public health-oriented diabetes prevention and control programs, and translating diabetes research findings into widespread clinical and public health practice.

Description: MEDLINEplus, sponsored by the National Library of Medicine, includes a vast amount of health information on over 600 diseases and conditions. The site offers authoritative, up to date and appropriate health information for both health professionals and consumers.

Information Available: Latest news concerning diabetes, publications from the National Institutes of Health, general information, alternative therapies, diabetes journals and newsletters, clinical trials, symptoms, prevention programs, research, statistics, law and policy, specific populations and reports on the management of diabetes.


National Institute of Diabetes & Digestive & Kidney Diseases (NIDDK)

Description: The National Institute of Diabetes and Digestive and Kidney Diseases conducts and supports research on many of the most serious diseases affecting public health. The Institute's Division of Intramural Research encompasses the broad spectrum of metabolic diseases such as diabetes, endocrine disorders, digestive diseases, nutrition, renal disease and hematology.

Information Available: Basic health information on diabetes, publications (available in Spanish), statistics, diabetes newsletter, as well as links to the National Diabetes Information Clearinghouse, the National Diabetes Education Program and a directory of diabetes organizations.

National Organizations:

The American Association of Diabetes Educators

Description: The American Association of Diabetes Educators is the leading authority in Diabetes Self-Management Training (DSMT) and in lifestyle management for the prevention of diabetes. The organization focuses on the goals of diabetes recognition, education, advocacy and research.

Information Available: Educational materials, publications, statistics, a legislative advocacy center and information about conferences and public educational campaigns.

Description: The mission of the American Diabetes Association is to prevent and cure diabetes and improve the lives of all people affected by diabetes.

Missouri Organizations:

Kansas City - Chronic Disease Coalition (KC-CDC)

Description: The KC-CDC is a community-based network that examines ways to promote health within the neigborhoods, churches and the community of Kansas City, Missouri. The KC-CDC works to address the high prevalence of diabetes and cardiovascular disease among African Americans and Hispanics. The organization hopes to create environmental conditions that promote widespread behavioral changes and improvement in population-level outcomes associated with the high prevalence of these diseases.

Information Available: The KC-CDC Action Plan, updates and background on the Health Walks project, community coalition and partnerships listings, program highlights and resources, success stories, and KC-CDC goals and objectives.


St. Louis Diabetes Coalition (SLDC)

Description: The SLDC was created to address the enormous challenge of diabetes awareness and care through an integration of efforts and resources. Representatives of SLDC include: managed care organizations, physician groups, state and community health programs, the American Diabetes Association, academia, business and professional organizations, hospital groups and pharmaceutical industry leaders.

Information Available: Diabetes guide, calendar of events, Diabetes presentations, active products and deliverables, press releases and media clips, research reports, SLDC community programs, SLDC committees and announcements.
