Patient-Centered Medical Home
In 2011, MFH launched its Patient-Centered Medical Home (PCMH) initiative to promote
partnerships among patients and their personal physicians. “Medical home” describes a
system of care where a primary care physician works with patients, families, and other
health care professionals to identify and access needed medical and nonmedical services.
This approach focuses on preventive care, management of chronic illnesses, and reduction
of the need for costlier care, such as emergency room visits and hospitalizations.
