Better Self-Management of Diabetes
Diabetes is a major health issue
in Missouri, ranking as the seventh leading cause of death in the
state. This chronic condition and its complications cost Missouri
a staggering $2 billion each year. Over 300,000 adult Missourians
have already been diagnosed with diabetes. A disproportionate number
of those are poor and from minority populations, and have the most
difficulty accessing health care.
Nationally,
self-management programs have shown the most positive results for
diabetes patients. It is important for people with diabetes, especially
the uninsured and underserved, to make and sustain life changes
in order to manage their condition. To do so effectively, they must
be educated, motivated, equipped, and actively supported in making
and maintaining those behavior changes.
MFH's Better Self-Management of Diabetes (BSMOD) is designed to
support the implementation of the Chronic Care Model's self-management
component at the primary care level. BSMOD funds organizations that
can help individuals diagnosed with Type II diabetes, particularly
those from high-risk, low-income, uninsured, underinsured, or underserved
